Liability Waiver
I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of  On Pointe Performing Arts Studio, LLC classes, rehearsals, performances, or activities. I also exempt, release, and indemnify On Pointe Performing Arts Studio LLC, its owners, agents, volunteers, independent contractors, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by On Pointe Performing Arts Studio, LLC. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold On Pointe Performing Arts Studio, LLC, its owners, agents, volunteers, independent contractors, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death.  I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.

Photo Release
Students of On Pointe Performing Arts Studio, LLC may have the opportunity to be photographed or video taped at various dance events. With permission from parents these photos or video may be included for On Pointe publications or advertisements, including on our Facebook page. To ensure privacy, specific names of children will not be included. No financial compensation shall be given for use of any photographs or video. I acknowledge that On Pointe may choose not to use my photo or video at this time, but may do so at its own discretion at a later date. On Pointe reserves the right to discontinue use of photos or video without notice.

Tuition Payment
I understand that all classes have a monthly tuition of the following prices: 30 minutes class = $10.50/class ($42/month); 45 minute class = $11/class ($44/month); 60 minute class = $11.50/class ($46/month). I understand that all classes meet once a week, except Intermediate Ballet. Intermediate Ballet is 90 minutes long, twice a week and costs $12.25/class ($98/month). Private lessons are 30 minutes long and cost $20/class ($80/month). Students taking private lessons have the opportunity of a solo dance at the year-end recital. Adult Dance Fitness has two sessions: (1) September to December, and (2) February to May. Class meets once a week for 30 minutes and costs $80 per session. Payment is due in full for Adult Dance Fitness. Students registered in multiple classes concurrently receive a discount of the following: 1st class = regular rate; 2nd class = $9.50/class (30 min), $10.00/class (45 min), $10.50/class (60 min); 3rd class or more = $8.50/class (30 min), $9.00/class (45 min), $9.50/class (60 min). A family discount is offered for families with multiple children taking dance classes concurrently. The family discount includes: 1st child = regular rate; 2nd child = $9.50/class (30 min), $10.00/class (45 min), $10.50/class (60 min); 3rd child or more = $8.50/class (30 min), $9.00/class (45 min), $9.50/class (60 min). If you are eligible for both discounts, you will automatically receive the discount giving you the most value. Payment is due the first week of the month via cash, check, or card. Credit and debit card transactions have a 5% convenience fee. Late payment will result in a $10 per month surcharge after the 15th of the month. After thirty (30) days of non-payment, students will not be able to participate in class or in a performance. There is also a one-time $25 registration fee each season (Adult Dance Fitness is excluded). Tuition and registration fee are non-refundable.

Recital and Costume Fees
Students will attend all scheduled rehearsals in the studio and/or in the location where performances are to be held. On Pointe Performing Arts Studio, LLC must be notified if a student will be absent at a rehearsal for the recital. Unexcused absences will not be tolerated, except in the event of an urgent personal emergency. Failure to be present for rehearsals shall be reason for suspension from the performance. No refunds are given for show dismissal. The dates of the recital and rehearsal are to be determined. Typically, On Pointe's recital is the first weekend of June. Costume fees can be paid in full or in three (3) installment payments (October, November, December). If a costume needs returned or is ordered late due to non-payment, I understand that I will be charged a shipping fee of $15.

General Student Policies
I have read and agree to follow On Pointe's Policy Handbook. I understand that my child is expected to attend class regularly and on time. If my child arrives more than 10 minutes late to class, she/he may be asked to observe class for their safety. Enrollment is assumed for entire season, so the studio must be informed of your withdrawal. I also understand that my child needs to follow the studio dress code listed in the policy handbook. It is my responsibility to have their hair up, properly dressed, and used the restroom before entering the dance room. I understand that I will need to sign-up for Remind text messaging / app to read all studio newsletters, emails, and other information in a timely manner to stay current on important information, changes, or deadlines at the studio.

By selecting the "I Agree" checkbox, I am signing this agreement electronically. I agree this electronic signature is the legal equivalent of my manual signature on this agreement. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
